I don't have a slide, so this is how I play it just using my fingers.
It probably has errors in it, especially with all the trills, so if
you can fix it, please do.

note:
   The Low E is tuned down to a D
   |--5^W--| -> bend a whole step
   |--3h5--| -> hammer on
   |--5p3--| -> pull off
   |--5/8--| -> slide
