Bit Part - Lemonheads

Saw that Bit Part wasn't on the archive, so I decided to figure it out and
post it.  I'm pretty new to the guitar, so this is probably off some from the
real chords used, particularly the Bm D in the verse/chorus.  I started out
playing the intro chord progression with bar chords, but I found that playing
the C-D slide as follows sounds cooler:
